We use this form to follow up on the information you provided at the beginning and in the middle of the year about each young person’s strengths and weaknesses in our target skill areas.  

We will ask you to answer questions about the impact of our program, and then ask you to re-visit the questions we asked you at the beginning and middle of the year about the student.

As the teacher perspective is a particularly important one to us, we ask you to think carefully about how this young person is doing at the moment.
